Welcome To
New Clients
A consultation fee of $50 will be needed to schedule. The consultation fee will be applied to the cost of service or refunded if no service is provided.
Cancellations
We request a cancellation at least 48 hours in advance to accommodate our high demand for appointments, as well as out of respect for all other patients. A $50 late cancellation fee will be charged for all cancellations under 48 hours.
How to cancel
You can cancel an appointment by calling 330-281-4699 or emailing [email protected].
Late Policy
Clients running more than 10 minutes late to their appointment may not be able to receive treatment and are subject to a Cancellation Fee of $50.
No Show Policy
In the event that a customer fails to show up for an appointment without canceling, they will be charged a “No-Show” charge of $100. Also, we will require that you make a $100 deposit towards any of your future services. A second no-show will result in discharge from our practice
Forms
Please have your forms filled out prior to your scheduled appointment time. If you are unable to fill out forms prior to your scheduled appointment please arrive 10 minutes early. If forms are not filled prior and you do not arrive to the office 10 minutes prior to your appointment. Your appointment may need to be rescheduled and a fee of $50 will apply.
